After your first cycle is likely when you can ask some questions as you so need to start somewhere, my understanding is all you can do is improve quality of eggs as best you can prior so I would take all supplements for 3 months prior to doing a first egg collection.
None of the questions below are to place blame on either person, it is more to explore if changes can be made to help which is easier if you have an idea of the main issues.
The questions I would have in my mine:
Did it seems like an egg quality issue or sperm?
Did the eggs inject well? Were they fragile looking?
What was our fertilisation rate?
Did the embryos have any fragmentation at any point? To note here that embryos can push mild fragmentation out but if the embryo is showing high fragmentation it might be more difficult.
If there is fragmentation should we consider the HALO test for the sperm to see how high the DNA damage is?
Things to to and introduce or ask about:
HIMMS for sperm selection where they use a "super magnifying glass" to select the best sperm
Should we add calcium activation- this is used to improve fertilisation rates so probably not needed if your fertilisation rate is good.
Do we need ICSI- or should we try IVF or do half and half?
Could we try the PEIZO procedure which is when they use a more blunt needle and it is supposed to be good for more fragile eggs. (We tried this once and did not have success so did not try it again).
